WTHS PROGRAMS

2008 REGISTRATION INFORMATION

2008 Player Fees:  $450

Three payment options are available to all players:

Option 1—CASH/CHECK PAYMENT IN FULL:

CASH/CHECK:  Payment in full at time of registration                                                               $450.00
Payment includes a non-refundable tryout fee of $75.00

Option 2—PAYMENT IN FULL PAYPAL:

PAYPAL:  Payment in full at time of registration via Paypal                                                        $465.00
Payment includes a non-refundable tryout fee of $75.00
and a $15 Transaction Fee

 

Option 3—THREE MONTHLY PAYMENTS—CASH/CHECK ONLY:

1^st Payment due at time of registration                                                                                             $250.00
Payment includes a non-refundable tryout fee of $75.00

2^nd Payment due April 1^st                                                                                                                    $100.00

3^rd Payment due May 1^st                                                                                                                      $100.00

 

The following forms should be completed at registration:

· Participant Registration Form

· WALA Waiver & Release Form

· WTHS Athletic Permission Form—a current sports physical form must be on file with the WTHS Nurse and WTHS Athletic Permission Form must be signed by a WTHS Nurse.

 

The following forms should be completed prior to the Player/Parent Meeting on February 28th:

· US Lacrosse Membership Application

· Player/Parent Handbook

· Player Fees Contract

· Fundraising Contract

· WTHS Uniform Contract

· Trainers Emergency Form

Equipment Requirements

All players should be prepared on day 1 of tryouts with the following equipment:

Lacrosse Stick                         Helmet                     Mouth Guard                           Shoulder Pads

Elbow Pads                             Gloves                      Athletic Cup
